Chondromalacia patellae (often abbreviated to CMP) was previously
called or meant as 'soft cartilage under the knee cap', and it affects
young and healthy athletes. The main cause of CMP is simple the
irritation underneath the surface of the kneecap. Most of the time, the
cartilage under the kneecap glides smoothly, but in some individuals,
the knee cap will rub against side of the knee joint, causing irritation
and inflammation in the cartilage surface, and causes pain to develop.
under the knee caps in common in young adults, especially those who
play soccer, cycles, rowers, volleyball players, runners, skateboarders
and ballet dancers - the pain from this condition is usually felt after a
period of immobility e.g. after sitting in for a movie. Skateboarders
often get this injury in their non-dominant leg because of the constant
kicking and twisting motion that it undergoes during skateboarding.
the condition may also be caused by a traumatic force, like an acute
injury e.g. a fall or a blow, or by chronic pressure at the patella and
the femoral groove where it passes during motion of the knee
flexion-extension. Some other possible causes and aggravators to CMP is iliotibial band (ITB) tightness, an unstable
core, malalignment of the knee forces as well as neuromas.
intervention begins with getting the patient to rest the knee, applying
cold therapy and compression as well as elevation to decrease
inflammation, swelling and pain; as well as pain and anti-inflammatory
medication from the physician.
After the acute
stage, we will start active physiotherapy intervention doing scar tissue
management, strength training as well as stretches to the muscle.
