The ankle is the place or the joint where the leg and foot joins or
meets. It is called the talocrural joint, and it is a synovial hinge
joint that connects both the distal end of the tibial and fibular bone
with the proximal end of the talus bone in the feet.
The bones are called the tarsal bones, and they are: talus, calcaneus (heel), navicular, cuboid, medial or internal cuneiform, middle cuneiform, and lateral or external cuneiform.
lateral malleolus of the fibular bone (feel the bone on the outer part)
and the medial malleolus (the bone protusion on the inner part), as
well as the bottom part of the distal tibia all jointly articulates with
the three (3) facets of the talus bone. They are all covered by
cartilages at each articulations.
joint is supported by the strong deltoid ligaments and the three (3)
side (lateral) ligaments - the anterior talofibular ligament, the
posterior talofibular ligament, and the calcaneofibular ligament. The
deltoid ligament supports the medial aspect of the joint, attaching to
the tibia's medial malleolus which connects to the calcaneus,
calcaneonavicular ligament, the navicular tuberosity and the talus.
joint is very stable with the foot in dorsiflexion (when the foot is
lifted up and towards the front part of the body), and when plantar
flexed, it is the most unstable. This is often when a sprain occurs. The
most commonly injured ligament is the anterior talofibular ligament.
The role of physiotherapy for a person with an injury to this joint includes minimizing pain by providing Ultrasound or Cold Therapy
to decrease the inflammation and provide pain relief. If necessary, the
physiotherapist may call in the hand therapist or occupational
therapist to fabricate a resting foot splint to place the ankle in 90 degrees to encourage healing in a good position.
the ankle is more stable and the inflammation has subsided, more active
physiotherapy intervention can be prescribed, including Manipulation & Mobilization, active Exercise and Movement, ongoing Electrotherapy to aid healing etc.
