Ankle sprains are one the most common injuries we treat in our
physiotherapy clinic. Most of the time, they are either caused by sports
injuries, or simply due to a slip. It is soft tissues damage around and
in the ankle, more of the time by an inversion injury where the ankle
is forcibly twisted inwards and into the body, or an eversion injury
where the ankle is twisted outwardly.
Due to the
structure of the bones in the ankle, most of the time, inversion
injuries occurs, and this type of ankle sprain results to injury and
damage to the lateral ligaments on the outside part of the patient's
Common symptoms following a sprain are as
followed, but note that the intensity or duration is dependent on the
severity of the injury.
- First degree sprain
- patient will experience pain when the foot is actively or passively
turned inwards and/or outwards. Damaged areas will be tender and
sensitive to touch.
- Second degree sprain - same as
above in terms of painful with motion, but the pain experienced will be
more intense; as well as moderate swelling and painful with walking
- Third degree sprain - often at this
kind of ankle sprain, patients will have excruciating pain and walking
is almost impossible. There is often accompanying substantial edema and
swelling, as well as a deformity if the ankle had been dislocated.
the acute stage of the injury the patient must not put too much weight
on the injured ankle, so walking on the ankle should not be done
whenever possible. We will train the patient how to walk with crutches
or walking frame with the injured ankle non weight bearing until it is
ready for any weights on it.
If a fracture is
suspected, we will inform the orthopedic surgeons to carry out an X-Ray
in an A&E (accident and emergency) department of your local
To relieve the swelling and pain, we may
use cold therapy; electrotherapy such as ultrasound therapy may be used
to bolster and accelerate the healing process of the injured ligaments.
Once the patient is allowed to weight bear and walk on the injured
ankle, we will start more active physiotherapy intervention.
