In sports, having a calf muscle injury is quite common. Sometimes
people calls it as a pulled calf or pulled leg muscle, and this
describes how the injury happened - the muscle being forcibly
over-stretched, tearing the muscle. A tear in the leg muscle is termed
as a calf strain, and it is classified into three categories (first,
second or third degree strains) based on their severity.
Depending on the level of strain, symptoms of a pulled calf is as followed:
- Grade 1
- signs and symptoms not so obvious, may be more pronounced after the
activity, or even the next day; commonly begins with cramping or a sense
of tightness, as well as a mild "tearing' pain as the muscles are
- Grade 2 - immediate presence of pain,
definitely more severe than compared to the pain in Grade 1. Tenderness
and soreness with palpation, and definite pain on stretch and
- Grade 3strains are very serious sports
injuries. Almost immediately, there will be sharp, stabbing or burning
pain, and the patient will be unable to walk without any pain. If the
muscle is completely ruptured, one can observe a large 'lump' or muscle
tissue on top of a hollow, soft depression where the tear is located.
1-2 days later, there will be a very large bruise because of the
localized bleeding inside the tissues. For this injury, there is a
definite need for surgical intervention.
Physiotherapy intervention for a calf muscle injury begins with allowing the patient to rest the injury, applying Cold Therapy compress plus elevation to decrease inflammation, swelling and pain. Sometimes, we need to fabricate a splint to protect the muscle from further damage.
the initial early and acute stage is complete, and when the patient had
spend sufficient time resting the injury, then more active
physiotherapy intervention can be started.
physiotherapy clinic, we will begin with gentle strengthening and
resistive exercises, as well as stretches to re-align the scar tissues,
to enhance healing and to increase the contractile strength of the calf
muscle. We usually first provide the gentle strengthening manually by
our physiotherapists, but as the patient's muscle gains strength, we
will use resistance bands to increase the ball game, and start with
other strengthening programs.
