Fibromyalgia refers to pain within the muscles and connective tissues, and is known as a medical disorder that has a chronic, global pain, with higher sensitivity to pressure. The symptoms may not necessarily be only pain, but also may cause severe fatigability, sleep dysfunctions and joint stiffness.

Some patients complains of difficulty with swallowing their foods, some has bladder difficulties, some has general numbness and tingling. It is often linked and co-exist with psychiatric conditions like depression and post-traumatic stress disorders (PTSD). Different patients has different experiences of fibromyalgia, not all experience the same symptoms. About 3% of the world's population has this disorder, with a female:male ratio of 9:1.

It is classified as a combination/either musculoskeletal disease or neuropsychiatric condition. The treatments that has been shown to be clinically effective include pharmaceuticals, cognitive behavioral management, patient and family education as well as exercise.

Signs and Symptoms

As the experience of symptoms by patients differ from patient to patient, it varies. The main defining symptoms includes chronic, global pain, fatigability and increased sensitivity to pain with pressure. Other symptoms that patients also reported numbness and tingling, consistent muscle contractions and muscle spasms, weaknesses in the extremities and body, twitching and 'jumping' of the muscles, bladder and bowel disturbances as well as sleep dysfunctions.

Many patients complain of a form of brain dysfunction, termed as 'brain fog', marked by poor concentration, short term- and long termed- memory loss, decreased reaction time, difficulty in multi-tasking, and poor attention span. On top of that, patients often has anxiety and depression.

Other linked symptoms include myofascial pain, also known as trigger points, which can cause localized or referred pain to nearby trigger centres, and they also may trigger a nervous system response.

Causes

  1. Genetic predisposition
  2. Stress may be an important 'tipping' factor
  3. Abnormal serotonin metabolism
  4. Deficient growth hormone (GH) secretion
  5. Psychological factors
  6. Physical trauma

Treatment

Treatments are segmented into three broad categories: pharmaceutics, investigational medicine and physiotherapy.

  1. Pharmaceutical - Lyrica, Cymbalta and Milnacipran are the three main medications that are FDA approved. Other medications that were often prescribed also include antidepressants, anti-seizure medication, dopamine agonists, muscle relaxants and opioids.

  2. Investigational medications - cannabinoids and the 5-HT3 receptor antagonist tropisetron.

  3. Physiotherapy - exercise increases fitness levels and regulates sleeping patterns, on top of reducing pain and fatigue in people with fibromyalgia.
