Lumbar spinal stenosis refers to the condition whereby the spinal canal space in the bones of your lower back becomes smaller and narrower than usual.
The main role of the spinal canal is to encase your spinal cord along the length of your back (something like an electrical casing for your wires in your house). When this space narrows, it crushes the nerves inside, and this can cause severe pain, numbness, weird nerve-related pains such as expericing tingling, heat or ice cold or more...and in severe cases, it can lead to weakness in your legs.
In very extreme cases, it also can cause you to potentially lose your bowel and bladder control (if the nerves controlling these functions are impaired).
So what happens when your spinal cord is compressed such as a lumbar spinal stenosis?
Most of the time, you will mainly experience pain. Depending on the level of pressure on your nerves, as well as the location (different location may cause sensation at different locations), so the symptom location, type and intensity will vary.
If it is mild, there can be just mild weakness and tingling....but if there is too much crushing pressure, it may block the electrical signals and impulses to and from the nerves to the leg muscles...and this can cause loss of movement and sensation. Sometimes, it can also cause loss of bowel, bladder and even sexual functions.
Most patients with lumbar spinal stenosis compensates to relieve their pain, and they find that bending forward really helps, so you can often find them bending forward as they stand, walk or sit. Bending backwards does the opposite - it causes more pain.
So why do our spinal canal even narrows?
Normally, our spinal canal naturally narrow with aging, resulting in degenerative changes. What this degenerative process cause is additional bone growth called osteophytes, growth in the size of our ligaments, or the decrease of spinal disc height as it loses water. All these add up to why lumbar stenosis is common in adults over 45 years old.
So what are our treatment options?
If the condition is very severe already, with weakness in lower limbs or loss of bowel, bladder or sexual functions, patients may need to immediately go for surgery to relieve the crushing pressure in the spinal canal.
For cases that are not so severe or immediately post-operatively, patients often undergo physiotherapy to improve the posture, strength and stamina of the supporting muscles in their lower back and abdomens; which mainly helps to reduce the amount of compression in their spinal cord.
Patients has to note that even after operation, treatment and self-management exercises, degeneration will continue as we age. We will continue to do our best.
