A mallet finger, also known as a baseball finger, or basketball
finger, dropped finger or generally as an extensor tendon injury, refers
to an injury to the extensor digitorum tendon of the finger(s) at the
joint at the tip of the finger(s) (distal interphalangeal joint - DIPJ).
It is caused by a strong force that causes a
more-than-usual-flexion (termed as hyperflexion) of the DIPJ of the
affected finger. Incidences where I've heard patients reporting to me
the causes includes hitting their finger against the cupboard door as
they're about to take their tea bags, being hit by basketball or
baseballs right on their finger tips, heavy objects falling on their
fingers and car doors or normal doors slamming on their fingers.
Most of the time, a total rupture of the tendon occurs, causing the droopy finger deformity to form.
Hand therapy and/or occupational therapy intervention commonly starts with pain relief approaches such as Cold Therapy and resting the injury, as well as elevating the hand to reduce inflammation and swelling.
Then we will Splinting and Braces the
finger in slight hyperextension to allow the ruptured tendon ends to
heal back better, and advise patient to let the finger rest and to not
move it for 6 to 8 weeks until the tendon joins back. If, at any moment
in this time, the finger is observed to be bent again, the 6-8 weeks
rest will start all over again, until the tendon is healed, or patient
opts for surgery.
After the mallet tendon finger has
healed, then we will start with more active physiotherapy, occupational
therapy and/or hand therapy interventions such as Exercise and Movement therapy (controlled and monitored) to improve range of motion, strength and dexterity.
