A meniscus tear refers to a tear in our shock-absorbing cartilage (the meniscus) in the knees. The meniscus is a C-shape fibrous piece of cartilage, which is only found in certain joints and it serves as a buffer between bones to protect the joint surface (kind of like a car's suspensions).
On top of serving as a shock absorber, the mensiscus also helps to lubricate the joint, and protects the joint by preventing over-flexion and over-extension.
Our Experience and Expertise
What our physiotherapists can do is to help diagnose meniscal tears by running a series of tests such as the McMurray's test and Apley's compression test - if there's pain in the inner part of the joint, that may indicate a medial (inner part) meniscus tear.
If there is anything unclear, we will help to refer you to a GP or specialists for an MRI to determine the extent of damage.
What are the typical symptoms of meniscal injuries?
Most of the time, meniscal tears are caused by a twisting motion or over-flexing of the joint. It's a common injury in sports that are characterized by rapid and forceful changes of direction (think football, basketball, rugby etc).
Sometimes one can hear and feel the "pop" sound during the injury, followed by lots of joint pain and swelling inside the knee. The discomfort increases when pressure is applied to the joint and a common issue that arises is a frequent "catch" or "lock" in the joint.
Physiotherapy Treatments and Intervention
The initial goals is to reduce as much symptoms as possible and protect the injured site from any further injuries or re-injuries as it heals. The initial stage management will include the common RICE protocol (rest, ice, compression and elevation) of the injured site - it's vital that this is done properly, especially by physios, to prevent bleeding and secondary tissue damage which will further slow recovery.
At this point in time, you are not encouraged to fully weight bear on your injured knee, you may even be required to use crutches to move about. Sometimes, you'll also be required to use a knee immobilizer to protect and prevent too much movement in the knees.
Your doctor may prescribe NSAIDs to manage the swelling and pain (NSAID refers to non-steroidal anti-inflammatory drugs).
What to expect with meniscal injuries
Depending on the severity, physical activities is allowed as tolerated, most of the time.
Your physiotherapist will guide and facilitate your healing through the process of regaining mobility and muscle strength on the return to full function and movement.
As and when necessary, we will refer you to an orthopedic consultant who can provide you medical advice, especially if you need a surgery. Take note that age does play a part in healing for treatment - younger patients heal faster but are likely to have problems without surgery.
Even though the knee is the most commonly affected meniscus in our bodies, yet there are other joints with similar structures (and injuries) such as the hip, shoulder and wrist joints.
They may be less common but they can be handled by our resident hand therapist.
