Patellar tendonitis, also known as jumper's knees, is yet another common cause of pain in the lower patellar area, especially in atheletes. It usually starts as light/small inflammation in the patellar tendon especially at the point of insertion into the patellar bone, and slowly worsens by tearing slightly at first, and gets worse later.

Patients often complain of aching, sore or cramping sensation and pain over the patellar tendon area, and most of our patients who has signs are about 10-20 years old. When we palpate the knee-patellar area, one can feel mild swelling and warmth, indicative of inflammation.

What Are The Causes?

Basically, it is an overuse injury caused by repeated overloading of the knee's extensor system; these causes microtears which healing rate can't keep up with the rate of new microtears forming by the repetitive loading. Patellar tendonitis happens to a range of athletes in many sports, of which includes bowling, football, volleyball, tennis, basketball, badminton, and even driving can contribute to the development of this injury.

Physiotherapy Treatment

During the early, acute stages, we will often use the RICE technique. RICE stands for Rest, Ice therapy, Compression and Elevation, which really means that the patient needs to rest and not aggravate the injury; icing to ease of any swelling and pain; compressing to press down the swelling and elevation to ensure the swelling doesn't escalate and becomes worse.

Later then we will prescribe specific exercise therapy as well as stretches to maintain the muscle, joint and tendon integrity and strength, that it wouldn't shrink as it rests.

There is a needling therapy that we do not provide in our clinic, as it's a more invasive approach to management. It is also known as prolotherapy (see diagram), and is used for patients whose pains levels has reached a level where function and mobility is impaired even after conservative measures. Please speak to your medical physician with regards to this approach.

Oftentimes, a combination of ultrasound and progressive strengthening, as well as deep tissue release to relax muscles bracing from the inflammation will be useful.
