Pelvic Floor Muscle Weakness and Physiotherapy

Weakness (medically termed as hypotonic) of the pelvic floor muscles aggravate to urinary incontinence and bowel incontinence, as well as organ prolapse.

When you have pelvic floor muscles that are weak/poorly controlled, you will need to consider going through a structured pelvic muscles strengthening program which is not "just doing a couple of Kegel exercises whenever you can". 

These are the common advice and instruction given to both men and women from their physicians. On a flip and extremee side, some patients are also told that they need to do a min number of 1000 Kegel repetitions a day...which is not rooted in exercise science.

Do note that the vast majority of patients and population cannot identify or locate these pelvic muscles without at least one consultation with a physiotherapist, as these muscles are not only hidden from view, but those who are untrained wouldn't know what to look for or what they're supposed to feel when they contract these hidden muscles.

The Cochrane Collaboration (a highly respectable committee dedicated to medical and health research and development) concluded in 2010 that patients who has any forms of incontinences (stress incontinence, urgency incontinence or mixed incontinence) should first see physiotherapists trained in pelvic floor rehabilitation / women's health physiotherapy, before seeking surgical solutions.

Possibly Able To Reduce Risks Or Necessity of Operations

Research shows that when you undergo a pelvic muscle strengthening program under the guidance of a trained physiotherapist, it is at least 80% effective as compared to doing it on your own. What this really means is that in every 10 patients that is recommended for surgery (such as sling repairs and TVT), actually only 2 actually require surgery after physiotherapy treatment.

In United Kingdom (UK), the National Institute of Health has issued a medical declaration that any individual considering operative or surgical correction for stress incontinence needs to first consult a pelvic muscle and women's health physiotherapist for conservative management and pelvic rehabilitation. Only patients who are unable to strengthen their pelvic floors enough with pelvic rehabilitation and exercises are then considered for sugery.

Surgical and operative approaches should always be approached cautiously and carefully, for surgeries do have their own set of risks for sling repairs.
