Posterior heel pain is not an actual diagnosis or condition, but really refers to a symptom of a condition. This pain is a very specific and particular pain that is just right behind your heel, and not below the heel.

This article focuses on posterior heel pain (which means literally, pain just right at the back of your heel).
This pain is commonly caused or aggravated by:
Bursitis - there are a few bursa located just behind our heel bone, which job is to protect this stress point. Bursa can be damaged such that they become inflamed and leads to bursitis and posterior heel pain.
Haglund's Deformity - refers to the enlarging or the upper and/or back portion of your heel. This enlarged bone then irritates the achilles tendon, causing inflammation, then swelling and pain.
Achilles tendinitis is the irritation, pain and inflammation of the large tendon at the back of your ankle. This condition is a very common repetitive strain condition that usually happens to athletes. This causes inflammation that causes sharp pain, especially in the beginning of the day as you get out of bed, swelling and even micro-tears in the tendon, making it very susceptible to full ruptures.
Usually, posterior heel pain is really caused by a biomechanical problem (means it's a structural, physical problem) in the feet, more than anything else.
Finding out the main cause of posterior heel pain can be quite tricky, as it can co-exist with other conditions that may too cause or aggravate this condition.
An example is that you may have posteior heel pain because of a heel spur, as well as an inflammation in your achilles tendon, and one of these two diagnoses can go unnoticed, untreated and you will have lingering pain.
What our physiotherapists will do when they see you for the first time is that they will assess you, diagnose your condition and find out the aggravating factors, then come up with an intervention and rehab plan for you, and start you on it. The plan will be mainly to firstly decrease pain, and removal of aggravating factors, so you will be pain-free.
