Scoliosis Physiotherapy and Rehabilitation

Scoliosis refers to a curvature in the spine can be a forward or sideways curve, that becomes obvious when you see the spine from behind. In many cases, symptoms are mild, but the severe version of this condition can be very debilitating and disabling.

Usually, it doesn't require any treatment per se until or unless it becomes painful or difficult to mobilize or function. It occurs primarily in the thoracic (the spine of the rib cage) and the thoraco-lumbar (spine of the rib cage and the lower back) region.

There are two types:

  1. structural (where the mechanics of the curve and the lateral curvature causes a pronounced one side of the patient's rib cage, very obvious when the patient is made to bend forwards). This is the worse kind, and can be progressive
  2. functional- fixed rotation doesn't happen, and any curvatures doesn't worsen. This form is more of a postural scoliosis, and seemingly disappears when patient is bent forwards.

Causes

Some of the common causes are as followed:

  1. leg length discrepancy, where one leg is shorter than the other.
  2. painful spasms of the back muscles.
  3. neurological and neuromuscular problems, such as birth defects, cerebral palsy, and muscular dystrophy can cause scoliosis.
  4. arthritis of the back and spine. It is a degenerative scoliosis causes by the gradual weakening of the soft tissues and ligaments in the spine.

Symptoms

Common telltale signs and symptoms include shoulders or waists that are uneven or unbalanced, the appearance of a shoulder or shoulder blade that is higher than the other, or one hip being higher than the other. If the scoliosis worsens, the spine will also twist on its axis, on top on the curving at the side, causing the ribs to appear more rotated and bent at an angle. In severe cases, back pain as well as breathing limitations will occur.

Scoliosis Physiotherapy and Occupational Therapy Interventions

For the rehabilitative management of this disease, here is a breakdown of what we will prescribe and do for patients.

Physiotherapy:

  1. exercise therapy to correct muscle imbalances
  2. correction of posture
  3. mobility equipment/aids prescription and training
  4. mobility and gait training

Deep Tissue Release:

  1. Regular deep tissue release will help to soften and loosen tight and shortened muscles for best results of posture correction

Occupational Therapy:

  1. ADL - activity analysis and modification, environmental (home or office or vehicle) assessment and modification,
  2. adaptive equipment prescription and training
  3. bracing, and using bracing during activities
  4. posture training in sitting, standing, and sleeping positions and in using positioning supports such as pillows, wedges, rolls, and corsets.
  5. productivity: electrical wheelchair or scooter prescription and training (mobility); work analysis and modification to improve productivity and output
  6. leisure: leisure identification and participation through the training and education on use of bracing, energy conservation techniques, activity modification

*results will vary depending on the severity of the curvature, maturity and coping ability as well as compliance of the patient, and the individual curve pattern
