Medically termed as stenosing tenosynovitis, trigger fingers and digits is a very common finger disorder, and its main characteristics is a clicking, catching, snapping or locking of the involved finger flexor tendon.

What happens is that the flexor tendon swells up and "catches" the retinacular pulley system (most commonly at the A1 pulley), causing difficulty in flexing and/or extending the finger, and the "clicking" phenomenon.

How it came to be called "trigger finger" is because as the affected finger unlocks, the finger retracts and "pops" back suddenly, similar to releasing a trigger on a gun. It also coincided with the time where many soldiers who actively uses guns, developed this condition, prompting them to call it as affecting their trigger finger.

Diagnosis is made mainly by two approaches:

  • subjective approach (clinical interview)
  • physical examination

You may find that more than one finger may be affected at a given time, but it usually affects the index finger, thumb, middle finger or ring finger (little finger slightly less occurrence).

The tiggering phenomenon is usually more common late at night and early into the morning, or as the patient grips an object firmly (such as holding a steering wheel, holding a knife to cut foods, gripping objects etc)

Medically, doctors will often prescribe a dose of anti-inflammatories first, before progressing to corticosteroid injection ("H&L"; hydrocortisone + lignocaine) which usually effectively lasts for weeks to months in more than 50% of patients who opted for the injection.

If the H&L injection proves to work only a short period of time, or if the patient prefers something more long lasting, physicians often prescribe a relatively simple surgical procedure (done on outpatient basis, under local anesthesia). The surgeon will cut the sheathe that is restricting the tendon, and the injury will be resolved permanently in the affected finger.

Unfortunately, the nature history of disease for trigger finger is still unclear and uncertain, though medical research has shown that trigger finger behaves differently in people with diabetes.

Also, it's not common for triggering to return after successful injection and even more rare after successful surgery.

Even though it may be painful and difficult to straighten the affected finger, and even though this phenomenon may persist for months, patients definitely benefits from consistent straightening exercises to stretch and straighten the finger.
